If you can answer "yes" to ALL of the following questions, please feel free to ask me for a letter of recommendation. Without the following, I will not be able to write a strong letter, so please have everything in place before asking.
- Have you taken a class with me in the past three years?
- Did you earn at least a "B+" in the class?
- Is the recommendation for graduate school or an academic-related scholarship/activity?
- Have you allowed at least 4 weeks between the date requested and date the letter is needed? ~ This is very important, a rushed letter will not be a good letter.
- Do you have a cumulative (or major) GPA of 3.5?
- DID YOU VISIT MY OFFICE HOURS AT LEAST TWO TIMES DURING THE SEMESTER WHEN YOU TOOK MY CLASS?
PLEASE PROVIDE THE FOLLOWING INFORMATION IF YOU ARE REQUESTING A LETTER
(Submit documents ALL TOGETHER to Dr. Evans online to drevans@ufl.edu): **Note, I will fill out EITHER the paper copies OR online recommendations, not both. Decide which you will request and let me know how to format/submit the letters. Be clear whether you are requesting online letters, if you will pick them up, or if you want them sent directly to the schools (...in which case, you must provide forms addressed & stamped envelopes for each school in advance).
EMAIL THE FOLLOWING INFORMATION TO ME IN A WORD DOCUMENT:
- A copy of your unofficial transcript
- An updated resume or curriculum vitae
- DETAILED information about the university/scholarship/program to which you are applying (supply a website link if possible)
- A copy of your completed statement of purpose/application letter
- Any program forms that must be filled out

- STAMPED AND ADDRESSED ENVELOPE(S) (unless requesting online rec.)
- A writing sample (preferably a copy of a final paper written in my class)
- A completed copy of the following questionnaire (the answers can be brief, but DO NOT put "see resume" as an answer).
- In a separate document: email a mock letter of recommendation. What letter would you write to the graduate/professional school committee if you were me? Include when you took my class, what assignments you completed, and your contribution to the class.
